The Medical Records Department is the heartbeat of the polyclinic. All clinical care starts and ends there.

How do I proceed after arrival at the polyclinic?

General enquiries: 

The telephone receptionist in the booth located at the entrance to the main waiting area will be happy to assist you.

Persons who have appointments to see the doctor:

  • Place your appointment card in the container according to the appointment time. (Booth No 2)

  • Have a seat and proceed to the Records Officer when your name is called.

  • After processing you will be directed to the General Practice area for screening by the Nurse.

 

 

Walk-in patients or persons who wish to make an appointment:

  • Take a number from the machine at the front of the Records Department. 

  • Have a seat and proceed to the Records Officer when your number is called.

Child Health Clinic Visits (Mondays and Thursdays):

  • APPOINTMENTS:

    • DO NOT take a number. 

    • Place your child’s immunization booklet or green card in the container provided according to the time of your appointment. 

 

  • NON-APPOINTMENTS or POSTNATAL BABY VISITS:

    • Place the book in the walk-in or postnatal container respectively. 

 

Please note that on Tuesdays, Wednesdays and Fridays parents and guardians are required to take a number in order for their child to be seen as a walk-in.

 

Persons with appointments to all other polyclinic services:

  • Proceed directly to the waiting area for the service.
