The Medical Records Department is the heartbeat of the polyclinic. All clinical care starts and ends there.

How do I proceed after arrival at the polyclinic?

General enquiries: 

The telephone receptionist in the booth located at the entrance to the main waiting area will be happy to assist you.

Persons who have appointments to see the doctor:

  • Proceed to the Records Officer in order of your attendance and in keeping with instructions of signage.

  • After processing you will be directed to the General Practice area for screening by the Nurse.

 

Walk-in patients or persons who wish to make an appointment:

  • Have a seat and proceed to the Records Officer in order of your attendance and in keeping with instructions of signage.

Child Health Clinic Visits (Mondays and Thursdays):

  • APPOINTMENTS:

    • Place your child’s immunization booklet or green card at the designated window in order of your attendance. 

 

  • NON-APPOINTMENTS or POSTNATAL BABY VISITS:

    • Proceed to the Records Officer in order of your attendance and in keeping with instructions of signage.  

 

Postnatal Clinic Visits (Fridays):

  • Proceed to the Records Officer in order of your attendance and in keeping with instructions of signage.

Persons with appointments to all other polyclinic services:

  • Have a seat and proceed to the Records Officer in order of your attendance.

  • After processing you will be directed to the waiting area for the service.
